WSIB Form 6 and the 1, 2, 3, 4 In Case of Injury Poster

Sometimes, a worker is injured despite training, legislative compliance, and strong safety culture. When that happens, what do you do? The WSIB (Workplace Safety Insurance Board) makes it as easy as 1, 2, 3, 4.  

What is the WSIB?

The WSIB website describes its services as “no-fault collective liability insurance.” They provide access to industry-specific health and safety information for employers; provide loss of earnings benefits and health care coverage for workers; and provide help and support for return to work after an injury or illness incurred by workplaces covered under the Workplace Safety and Insurance Act. The WSIB is entirely funded by employers.

​To make a claim under WSIB in the event of an injury, employers must follow a specific process for reporting injuries. The In Case of Injury: 1, 2, 3, 4 Poster outlines this process. Displaying this poster in the workplace is a mandatory posting requirement. 

In Case of Injury

​Step 1: Get medical help
Your employer is responsible for providing first aid. Go to the doctor or hospital if you need treatment. Your employer pays for your transportation.
 
Step 2: Document
Tell your employer about your injury or illness. They investigate and keep a record of what happened.
 
Step 3: Report to the WSIB
Employers must tell us within three days if an injury or illness happens. You can report by submitting Worker’s Report of Injury/Illness (form 6).
 
Step 4: Work together
We work with you and your employer to help you recover and return to work safely and at the right time.

Fill out form 6 (or form 7 if you’re the employer) to report a work-related accident to the WSIB if you require health care and/or:

• Are absent from regular work
• Earn less than regular pay for regular work
• Require modified work at less than regular pay
• Require modified work at regular pay for more than seven calendar days

To file your form 6 for the WSIB, you’ll need the following information:

1. Worker information (name, date of birth, address)
2. Employer information (name, address)
3. Accident/Illness details (date of accident, area of injury)
4. Health care information (treatment date & location)
5. Employment information (work schedule, earnings)
6. Return to work information (modified work details)

 
After you submit this report, you'll receive a 4-digit confirmation number. There is no need for you to fax or mail another copy of this report to the WSIB.
